Responsibilities:nnResponsible for completing MDS assessments and MMQ's and updating care plans according to MDS schedule.nEnsures timely and accurate documentation for level of care providednCompletes all state forms as required for reimbursement purposesnFollows RAI/MDS schedule for Long-term and Short-term residentsnReviews Kardex and Care Plan with each RAI/MDS assessment and MMQ completionnOversees care plan revisionsnInputs MDS data into systemnSubmits MMQ's according to state regulations.nSubmits data to Federal and State programs within time frame allowednCompletes any additional assessments as indicated & functions as a resource, providing services as appropriate to ensure all staff have a working knowledge of the care plan.nnEducation/Experience/Licensure/Certification: nnRN currently licensed to practice in the State of MassachusettsnTwo years full time or full time equivalent clinical experience with experience as a MMQ/MDS NursenExperience and/or special interest working with older adultsnAble to perform tasks which may be physically demanding such as pushing, bending and lifting up to 50lbs.nAbility to maintain positive working relationships with residents, their families, peers and other staff membersnIn good physical and emotional health and free of communicable diseasesnn Maplewood Senior Living conducts pre-employment screening including background check, drug screening and reference checks.n Due to Department of Public Health requirements, all candidates will be required to show valid proof of full vaccination for COVID-19 (1st vaccine, 2nd vaccine, and booster) before commencing work, unless they receive an approved exemption from Maplewood. The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
